New Official Application Form - This form should only be used by officials who did not register Emerald Coast Officials as his/her primary association with the FHSAA for the previous season.

Official Evaluation Form (Online Version) - This form is to be used to evaluate all officials of a crew during a single contest.  You may not evaluate yourself or your partners.  You may only evaluate the officials from a spectator's point of view.

Official Evaluation Form - Simplified (Print Version) - This form is to be used to evaluate all officials of a crew during a single contest.  You may not evaluate yourself or your partners.  You may only evaluate the officials from a spectator's point of view.  Mail or fax the form to ECO upon completion.  (Microsoft Word Required.)

FHSAA Forms - These forms include: Request To Take Contest Officials Make-up Rules Examination, Appeal of Disciplinary Action By Contest Official, Unsportsmanlike Conduct Incident Report, Exceptional Sportsmanship Report, and Field Clinic Application.  (Adobe Acrobat Reader and/or Microsoft Word required.)

Coaches/Captains Pre-game Meeting Card - This card is to be read to the coaches and captains during the pre-game meeting.  (Adobe Acrobat Reader required.)

2 Official Pre-game Conference - This is to be used as a guide when conducting a two-official crew pre-game conference.  (Adobe Acrobat Reader required.)
