Official Evaluation Form

Instructions:  You may evaluate as many officials during the season as you wish.  You may not use this form to evaluate yourself or partners during a game.  This form is to be completed from a spectator's point of view only.  This form must include all members of the officiating crew.  Rate each official according to the following rating system.  You must provide comments if a rating of three (3) or lower is given.  Your name will not be disclosed to the officials you are evaluating. 
Rating System: (5) = Best of the best.  Needs no improvement.  (4) = Very good.  Room for a little improvement.  (3) = Average.  Considerable room for improvement.  (2) = Below average.  Needs much improvement.   (1) = Terrible.  No where to go but up.
